Forms
The Dispute Board Federation provides various sets of forms which it hopes will be useful to both contractors and owners in the development of projects. These forms include standard form Dispute Board formation agreements, Disclosure forms and checklists for Board Members, Requests for the Appointment of Board Members, and similar documents.
In addition form Recommendations and Decisions are also included for use by either Dispute Board members, contractors and owners.

The Dispute Board Federation also provides reference forms for use by individuals, institutions and others in the proper use of Dispute Boards, their operation and procedures.
